Physical exams are REQUIRED for grades nursery, pre-K, K, 1, 3, 5, 7th grades and new students to St. Raymond's. Dental completion slips are RECOMMENDED for these same grades. If your child has an early birthday, please download the health assessment form and have it filled out by your health care provider. Return it to the Health Office as soon as possible. Physicals are now good for one year, as long as they are dated after the first day of the previous school year. Since insurance companies will only allow one exam per year, this makes it easier for parents to obtain them.

ALL medications to be given by the school nurse MUST have a written health care provider's order. This includes over the counter meds such as tylenol, motrin, etc. Medication cannot be administered without a healthcare provider order-It's the law! Statistics for New York State are kept on communicable diseases. If your child has any of the following please call and notify me: chicken pox, scabies, flu, ringworm, fifth disease, mono, pink eye (conjunctivitis), strep throat/scarlet fever, and the infamous lice! You also MUST notify the Health Office if your child will not be in school that day. We don't know if your child arrived safely to school or not if they aren't in the building. The phone is on 24/7 and you can leave a message anytime. If your child is absent for 3 days or more they MUST have a doctor's note and a parental absent note to clear them to return to school.If they have a fever, they must remain fever free for 24 hours without taking a fever reducing medicine. Antibiotics must be given for 24 hours before returning to school. Thank you!
